From your description it sounds like the coolant temp sensor is missing on your new throttle body. I think that is what that grean sensor was on your old one. That could be causing your cold start problems because the ECU doesn't know it is cold and won't richen the fuel mixture properly.

The wires for the spacer under the TB I think is a heating element for cold starting. I do not know how to hook it up. Maybe you can connect it with a switch to power and ground and turn it on for cold start up?

yep, the black deal under the throttle body is an electrically operated heater on the newer cars.
